Description
This project will advance equity in healthcare practice and improve the quality of care for people with complex health and social needs (complex populations) by working with a range of stakeholders, including community members, to develop and disseminate a comprehensive curriculum for healthcare providers in institutional and community settings. Complex populations too often receive uncoordinated care that fails to address their range of needs, exacerbated by healthcare-system racism, leading to mistrust and devastating outcomes for individuals and communities. Major deliverables will include: (1) working with consumers and providers to develop a structured curriculum with the skills and knowledge healthcare providers need to deliver equitable and patient-centered complex care; (2) creating online courses for continuing-education units and a full certificate course in complex care; (3 launching synchronous/asynchronous courses on the website of the National Center for Complex Health and Social Needs; (4) promoting course uptake through national and regional organizations' networks; and (5) monitoring impact.
