Supporting the Aligning Forces for Quality Consumer Survey in three communities, Round 2

The Foundation's initiative, Aligning Forces for Quality: The Regional Market Place, was designed to help communities across the country align health care forces to improve the quality of health care.This project will administer a follow-up survey of consumers in three Aligning Forces for Quality communities. Deliverables will include a community-specific report for each of the three sites and a public-use dataset for inclusion in the Robert Wood Johnson Foundation's Health and Medical Care Archive.
