Conducting the history and physical exam: A neurorehabilitation consultant guide

Abstract

Taking a careful and complete history, combined with a thorough physical and functional examination, are hallmarks of excellent physiatry consultations. Home environment and functional histories in mobility, activities of daily living, instrumental activities of daily living, and cognition/communication are unique elements of the neurorehabilitation consultant's history and physical assessment. A neurological examination permits the interpretation of anatomical abnormalities revealed by magnetic resonance imaging and computed tomography of the brain and spinal cord. A thorough neurologic and musculoskeletal exam is essential in eliciting impairments, developing a differential diagnosis, and determining a rehabilitation plan. The neurorehabilitation consult history and physical goes beyond the standard medical assessment, focusing on elements relevant to functional impairment and disability, which are critical to formulating an appropriate individualized plan of care.
