Abstract

BACKGROUND: Gestational diabetes insipidus is a rare endocrinopathy caused by elevated levels of vaso-pressinase, with a paucity of cases described in the obstetrics literature. CASE: We present 2 cases of women who presented in the third trimester of pregnancy with complaints of severe polydipsia, polyuria, and nocturia. Maternal laboratory work was remarkable for decreased urine osmolality and increased serum osmolality. Both patients were effectively treated with desmopressin, which improved maternal symptoms. Both women had resolution of the diabetes insipidus in the postpartum period. CONCLUSION: While rare, early recognition and management of gestational diabetes insipidus can avert potential maternal and fetal harm.
