Abstract
Maintenance of nutritive cerebral perfusion is the essence of providing care to the patient with neurosurgical disease. Many neurosurgical problems include elements of intracranial hypertension. The dual nature of cerebral perfusion with elevated ICP, oligemic versus hyperemic, can lead to disparate therapeutic strategies. These issues and others with conflicting therapeutic priorities are discussed in this chapter.
