Mechanical heart support in patients with terminal heart failure

Abstract

The insufficiency of blood circulation is developing at a certain stage of a current of the majority of diseases of heart, is the progressing process, being accompanied a high invalidization and a lethality. The main method of treatment of patients with terminal heart failure is heart transplantation. But the quantity of necessary donor organs catastrophically doesn't suffice even in the countries with high level of transplant activity. Use of devices of the auxiliary blood circulation (artificial ventricles of heart) can be the main alternative of transplantation. In Scientific Research Institute of Pathology of Blood Circulation longterm mechanical support was applied at 18 patients (16 men and 2 women) with heavy stagnant heart failure of FC IV on NYHA, refractory to drug therapy. In 14 cases the dilatational cardiomyopathy, and in 4 cases after heavy myocardial ischemia was the reason of heart failure. Biventricular rounds of heart (EXCOR system) were applied in 6 cases. Our experience shows that implantation of long-Term mechanical heart support systems of with can serve in patients with progressing heart failure is effective bridge to heart transplantation, but is accompanied by danger of terrible complications.
