Abstract
Parotidectomy can encompass removal of the superficial lobe, deep lobe, or the entire gland to remove benign and malignant masses involving the gland. Occasionally, the overlying skin is involved by tumor extension from the gland parenchyma and requires resection. The resultant volume loss, with or without a concomitant skin defect, can lead to a significant cosmetic deformity. For larger masses, the volume loss alone, if not reconstructed, will result in a noticeable contour defect that over time can lead to an inferior rotation and contracture of the ipsilateral ear. Several methods to reconstruct these defects exist and this chapter will focus on the use of the submental artery island flap to restore volume and skin. It has the advantage of being able to be incorporated into a traditional modified Blair incision and minimizing operative time and donor site morbidity as compared to free flap reconstructive options.
