Service in the United States Army Reserve (USAR) offers surgeons a unique and rewarding opportunity to contribute to military medicine while maintaining a civilian career. This chapter provides a guide to navigating a dual career in the USAR, covering accession pathways, training requirements, and unit assignments. It emphasizes the importance of engagement, professional military education, leadership development, and operational experience for success. Key topics include balancing military and civilian responsibilities, understanding training structures such as the Direct Commission Course (DCC) and the AMEDD Basic Officer Leader Course (BOLC), and exploring assignment options through Troop Program Units (TPUs), the Army Medical Department Professional Management Command (APMC), and Individual Mobilization Augmentee (IMA) billets.
