Abstract
Clinicians who provide care in the hospital are often faced with creating a comprehensive plan of care for a patient/family living with a life-limiting illness. Understanding the hospice and palliative medicine resources available will help clinicians to create a patient- or family-centered discharge plan based on the patient/family goals of care. Hospitalists must understand the importance of a well-documented discharge care plan with goals of care and symptom management that is sustainable and possible to be carried out at the site of discharge.
