Pathophysiology of obesity after spinal cord injury

Abstract

The accumulation of body fat by persons with spinal cord injury (SCI) has been underappreciated by clinicians and epidemiologists because of the failure to recognize limitations in the use of body weight and body mass index to characterize obesity in this special population. The purpose of this article is to more appropriately define adiposity in SCI, review the pathology of obesity, and discuss the factors that uniquely predispose persons with SCI to become obese. The link between adiposity and defining characteristics of the metabolic syndrome will be clearly demonstrated, including the role of central adiposity on dyslipidemia, vascular inflammation, hypertension, and insulin resistance.
