Abstract

Advanced heart failure is not a rare sequela of acquired or congenital heart disease in children. As medical therapy provides insufficient support for many patients with advanced heart failure, mechanical circulatory support (MCS) plays a prominent role in both short- and long-term cardiac support. This chapter will review the etiologies of advanced heart failure in children and the important determinations of patient selection and comorbidities for MCS. Devices currently employed in children include extracorporeal membrane oxygenation, temporary ventricular assist devices, and durable devices. These will also be reviewed in detail, as will considerations for postoperative management and transitioning patients to home care on ventricular devices.
