Abstract
Surgical management of pilon fractures remains challenging, with treatment continuing to evolve. Although the two-staged approach is widely recommended, soft-tissue complications are problematic. New region-specific implants and new surgical techniques have been developed and have improved treatment of this difficult fracture. Definitive external fixation also has had strong proponents. Sound surgical judgment of the type of injury is still the most important tool in obtaining optimal results.
