Abstract
A 13 year old boy was taken up for surgical correction with a diagnosis of Ostium Primum atrial septal defect and persistent left superior vena cava. However, during surgery other anomalies in the form of unroofed coronary sinus and a double mitral orifice with cleft anterior mitral leaflet of the major orifice were detected. The patient underwent uneventful surgical correction and continues to remain well during a follow up of two years.
