Abstract
Neoadjuvant Chemoradiation may lead to tumor downsizing and downstaging. In selected cases when tumor regression is so significant that no tumor can be found on clinical and radiological examination, it has been our practice to include these patients in a strict follow-up program with no immediate radical surgery (Watch and Wait). In this chapter we present the rationale, selection criteria, tools and follow-up algorithm of this strategy along with de pitfalls encountered after more than 20 years of experience.
