N1 - Funding Information: The Washington Health Alliance was formed in 2004 from an effort led by the King County government to address the rising cost of health care through a communitywide approach. A purchaser-led multistakeholder organization, the alliance’s participants include public and private employers, union trusts, health plans, health systems, physicians, community organizations, consumers, and others. Its major QI activities under AF4Q include creation of a Web-based portal to provide performance measurement reports to clinics; a peer-to-peer learning network conducted in partnership with the state medical association and Academy of Family Physicians; a 32-month multipayer medical home payment pilot; a collaborative, established by the state legislature, to identify and recommend evidence-based strategies; and support of the state’s transitions of care pilot program, which is funded by the CMS Partnership for Patients program.
