note = "Funding Information: The Washington Health Alliance was formed in 2004 from an effort led by the King County government to address the rising cost of health care through a communitywide approach. A purchaser-led multistakeholder organization, the alliance{\textquoteright}s participants include public and private employers, union trusts, health plans, health systems, physicians, community organizations, consumers, and others. Its major QI activities under AF4Q include creation of a Web-based portal to provide performance measurement reports to clinics; a peer-to-peer learning network conducted in partnership with the state medical association and Academy of Family Physicians; a 32-month multipayer medical home payment pilot; a collaborative, established by the state legislature, to identify and recommend evidence-based strategies; and support of the state{\textquoteright}s transitions of care pilot program, which is funded by the CMS Partnership for Patients program.
