The role of antibiotic alternatives in controlling multi-drug resistant wound infections

An injury to skin initiates an array of pathophysiological events, including inflammation, tissue remodeling, and tissue repair. However, microbial colonization and invasion of the exposed subcutaneous tissues with pathogenic microorganisms increase the risk of wound infections. The threat is even greater when pyogenic wounds are colonized with antimicrobial resistant pathogens. With almost no new class of antibiotic drugs in the development pipeline, the use of non-antibiotic antibacterial agents, including phytochemicals and metals have extensively been researched to replace or supplement the current arsenal of antimicrobial medications. Furthermore, the application of phages, the natural bacterial predators, has been found to be effective in resolving chronic multi-drug resistant wound infections. Recently, the primary objective of wound management has been revised to address the role of skin microbiome in acute and chronic wound healing. In light of these new paradigms of microbial ecology and beneficial bacterial interactions, probiotics therapy have also emerged as potential pro-healers aiding tissue repair mechanisms associated with skin abrasions. This chapter will discuss in detail the various antibiotic alternative therapies that could be used against antimicrobial resistant bacteria in wound infections.
