Abstract
A patient with dysphagia showed an unusual position and shape of the stomach during a gastric emptying study. A transmission image that was obtained by placing a flat Co-57 source behind the patient permitted unequivocal diagnosis of a hiatus hernia with marked food retention, which was later confirmed by other imaging modalities. This index case provides strong support for the use of transmission imaging for anatomic definition in unusual patterns observed on gastrointestinal scintigraphy.
